  First Time Patients
What to Expect on your first visit
In order to provide a holistic approach to your health, your first office visit will take approximately 90 minutes. This provides enough time for a comprehensive review of your overall state of health, answer questions, discuss additional testing if applicable, and perform a physical exam. We will work with you to start your journey to total wellness by designing a practical yet effective individualized treatment plan.

Please download the New Patient Intake Form and complete the form in advance of your appointment. Please fax the forms to our office at least 2 days before your appointment. If it is not possible to send us the form in advance of your appointment, please bring the completed forms, copies of any lab results, and other diagnostic tests with you to your first visit. Your Return Office Visit
In your follow-up appointments, your progress will be evaluated and changes to your treatment plan will be implemented as necessary. The average follow-up consultation is 30-60 minutes.

Payment Policies
In our continued commitment to provide the highest quality of care available to all of our patients and to have those services comfortably affordable, payments are due at the time of service. We accept payment by cash, check or credit card (Visa or MasterCard).

Insurance
• Office Visits
  We do not directly bill insurance for medical services provided at Ocean Pacific Integrative Health Center. However, we can provide you with a coded claim (superbill) to submit to your insurance company for reimbursement. Please contact your health insurance company for more detail about coverage for services provided by a “Licensed Naturopathic Physician”. Some PPO plans may provide partial to full reimbursement for services provided at our office.

• Labs and Diagnostic Imaging
  Labs ordered at our office are typically covered by your insurance company. Please contact our office at 760-944-9300 for more information.
